Building a Culture of Safety

Patient safety is critical to our goal of providing excellent clinical care. Creating a culture of safety is one way that Gwinnett Medical Center has worked to ensure that safety is first and foremost in the way we provide care and services.

Building and supporting a culture of safety is a priority at all levels of our organization.

Reporting of Actual and Potential Errors
Mistakes can and do happen in health care. Such mistakes can have tragic results. Our system is challenged with identifying and addressing potential errors before they reach a patient.

Staff and physicians must feel free to report unsafe conditions as they are discovered. We strive for a non-punitive, proactive, team approach to patient safety. While it is a natural reaction to blame a person who made an error, overwhelming evidence points to the fact that the staff member isn't the problem but instead that mistakes are caused by flawed or poorly designed systems and processes.

When errors do happen, we aggressively evaluate the error and correct identified problems to reduce the likelihood that the problem will recur. From a system perspective, we are also working to make error reporting more efficient and easy.

We also are committed to informing patients and/or their families when an error in their care has been discovered. This commitment to disclosing medical errors reflects our core values and is fundamental to maintaining our position of trust with our patients and the community we serve.

Education and Training
We are committed to providing safety training to all staff specific to their job responsibilities. Managers receive targeted training to assist them in creating a unit-level culture of safety, and to support them in their goals of reducing errors in their areas.

Evaluating Our Progress
We want to know how our patients, employees and medical staff feel about safety in our organization. That is why we formally survey each group at regular intervals to determine their perceptions of our safety culture, and our progress in meeting our safety goals.

Top organizational leaders also participate in routine patient safety rounds in clinical areas to demonstrate their commitment to safety, and to obtain informal feedback from front-line staff about ways to improve safety. Suggestions to improve safety are welcomed and implemented whenever feasible.
