STEP 3: Orientation Requirements
Faculty and Student Orientation Materials
On-site faculty and students will need to read and complete the information listed on this page as it applies to their role at Gwinnett Medical Center (GMC). After identifying your role, select the appropriate category below (i.e. identify your role within a non-nursing or nursing category), click that link, and print the document to help guide you through the GMC requirement process. Then proceed to complete appropriate Orientation Requirements numbered 1-8.

Student (Nursing)Student & Faculty Orientation Requirements
1.Read and study the following orientation materials. Print and complete the accompanying survey questions and test. Remember to put your name on these documents.
- Orientation Manual Infection Control
- Tuberculosis
- Orientation Survey Questions and Test Orientation Survey Questions and Test Answer Key
2. Read and study the Information Privacy & Security Module, print the documents below and take the test.
- Information Privacy & Security Test and Answer Sheet: Put your name on the test and anwer each question.
- Give your test answers along with the signed Confidentiality Agreement Form to your instructor.
- Faculty correct the test with the following answer key: Information Privacy & Security Answer Key
3. Print, read, and study the Code of Conduct
- Print and sign the Receipt and Acknowledgment of Code of Conduct and give it to the GHS Coordinator.
4. Read and study the Corporate Compliance Module, print the documents below and take the test.
5. If students will be expected to do patient tests for glucose you will receive individual training on the PCX or PXP Glucose Meter either from your instructor or other competent GMC trainers. NOTE: Students will use their instructor’s bar code for the “Operator ID”. Once on site review the Glucose Policy on Gwinnetwork (GMC internal website). The following forms pertain to the meter:
6. If students will be observing or in a surgical tech role in the operating rooms, read and study the GMC Student Protocol For The Operating Room module. Then complete one of the following as applicable:
• Student observers:The Student’s instructor signs the GMC Student Observers Form and sends signed form to Linda Horst in Learning Resources, MOB 100, Suite 115 or FAX: 770-682-2220.
• Student Surgical Techs:o The student’s surgical tech instructor signs the Student Surgical Tech Form and interoffice mails the signed form to Learning Resources, MOB 100, Suite 115 or FAX: 770-682-2220.
o The student prints the Student Surgical Tech Preceptor Checklist Form and has their OR preceptor sign and interoffice mail completed form to Learning Resources, MOB 100, Suite 115 or FAX: 770-682-2220.
7. Obtain a Instructor/Student ID Badge to wear in addition to your school ID Badge. The Badge will allow you to park at Gwinnett Medical Center. The cost is $5.00 and is refundable upon returning the badge. Click here to complete the ID Badge form. Instructors may go to Security to obtain badges (students cannot obtain their own badges).
8. Instructor turns in “Student Log for Documenting Placement Requirements” documenting that their student/s have completed all the necessary requirements.
