Bridging the Gap

After an acute illness or injury, you may find that you are not well enough to be cared for at home. The Gwinnett Extended Care Center can bridge the gap between levels of care while accommodating individuals who need subacute or intermediate care.
 

Conveniently located on the Gwinnett Medical Center campus, the Gwinnett Extended Care Center is a 33,375 square foot facility with nine private and 40 semi-private rooms.  Care is delivered by an interdisciplinary team, with services including nursing, rehabilitation services, respiratory therapy, wound care, social services, nutritional support, and therapeutic activities.

 

 

Levels of Care

 

The Gwinnett Extended Care Center provides various levels of quality, cost-effective care for those we serve.  Physicians, therapists, nurses and other health professionals provide highly skilled services with a strong focus on family involvement.

 

Subacute care is designed for those individuals who have experienced an acute illness or injury.  This level of care is considered short stay and focuses on transitioning the individual back to the community.

 

Intermediate care is designed for individuals who require a lower level of care.  This care may include providing assistance with activities of daily living for patients who are no longer able to return to the community on their own.

Our primary goal is to help our patients achieve their highest level of functioning.  One step toward reaching this goal is to provide a smooth transition from the hospital or home to the Gwinnett Extended Care Center.  A physician and our interdisciplinary team will work together to address each individual need.

 

At the Gwinnett Extended Care Center, we strive to provide a comfortable environment conducive to wellness.  Patient rooms are designed to create a homelike atmosphere.  An enjoyable dining facility and an interior courtyard with walking paths add to our pleasant surroundings.
