Advance Directives and Living Wills
Gwinnett Medical Center supports the rights of patients to make informed decisions regarding their medical care.
An Advance Directive explains how you would like your health care handled if you are no longer able to speak for yourself. If you already have an Advance Directive/Living Will, please bring a copy with you when you are admitted to the hospital so that you can discuss what your wishes are with your health care team.
Your Advance Directive will be scanned and a copy will be attached to your medical records. However, if you do not have an Advance Directive and wish to obtain information, we can provide you with the information and the explanation needed to make an informed decision at no charge. You can also download a copy of an Advance Directive.
