Advance Directives and Living Wills
Gwinnett Medical Center supports the rights of patients to make informed decisions regarding their medical care.
An Advance Directive explains how you would like your healthcare handled if you are no longer able to speak for yourself. If you already have an Advance Directive/Living Will, please bring a copy with you when you are admitted to the hospital so that you can discuss what your wishes are with your healthcare team.
Your Advance Directive will be scanned and a copy will be attached to your medical records. However, if you do not have an Advance Directive and wish to obtain information, we can provide you with the information and the explanation needed to make an informed decision at no charge.
